An adult at risk is a person aged 18 years or over who has needs for care and support. Any agency providing care or any member of the public can complete this form, please complete it with as much information as is available.
A Safeguarding Adults Review (SAR) is an independent review of how agencies worked together to safeguard an adult who has died or been seriously abused or neglected. Find out more about the process and how to make a referral here.
