Why do you need my information?

The purpose for collecting data, as and where appropriate, is for the delivery of Knowsley’s Safe & Steady Exercise Programme. Anyone accessing this service may be required to share information with the service provider for the purpose of delivering the service and supporting the individual accessing the service. Knowsley Council have commissioned Everton in The Community to deliver this service.

What personal information will be processed?

Our commissioned service provider, City Health Care Partnership, will process the following personal information:

Personal information

  • Names (forename and surname)
  • Addresses
  • Telephone numbers (landline/mobile)
  • Email addresses.

Special Category Data

  • Health information in relation to history of falls / mobility of individuals.

What allows you to use my information?

Knowsley Council commissions the Safe & Steady Exercise Programme as part of the falls and fracture prevention pathway. In line with the UK General Data Protection Regulation (UK GDPR) any personal data that is processed by Knowsley Council will be in line with line with the following lawful bases:

  • Article 6(1)(e) of the UK General Data Protection Regulation (UK GDPR), which states that the processing of personal data is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller.
  • Public Health (Article 9(2)(i)): Processing is necessary for public interest in health, such as managing infectious diseases.

Knowsley Council will process any special category personal in line with the following lawful basis under the UK GDPR:

  • Article 9(2)(h) of the UK GDPR, which states that the processing of special category personal data is necessary for the purposes of the provision of health or social care.

The relevant lawful condition for the processing of special category personal data under the Data Protection Act 2018 is Schedule 1, Part 2, Paragraph 2, Health or social care purposes.

Who will my information be shared with?

Aggregated and anonymous data may be shared by the service with Knowsley Council as part of monitoring arrangements to support the delivery of the service. The Council will not share the data with any other third party.

Do I have to provide this information and what will happen if I don’t?

No, individuals are not legally required to share information with the service. You can decide whether to share data or not with Everton in the Community, and you can decide how much or how little you wish to share.

How long will you keep this data for and why?

Anonymous data will be held by Knowsley Council for a period of up to 5 years. Any personal data processed by Knowsley Council will be held in line with our Records Management Policy and Retention Schedule This is available upon request.

How will my information be stored?

Data held by Knowsley Council will be stored on a secure electronic system. Access to these systems is restricted to those who are entitled to view and process your data.

Will this information be used to take automated decisions about me?

What rights do I have when it comes to my data?

Under the Data Protection Act (DPA) 2018/ UK  GDPR, you have the following rights with regards to your personal data: -

  • The right to rectification – you have the right to ask the council to correct or remove any inaccurate data that we hold about you.
  • The right to erasure (right to be forgotten) you have the right to ask the council to remove data that we hold about you.
  • The right to restriction – you have the right to ask for your information to be restricted (locked down) on council systems.
  • The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request.
  • The right to object – you have the right to ask the council to stop using your personal data or to stop sending you marketing information, or complain about how your data is used.
  • The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable).
