Who will own my data once I submit it?

Why do you need my information?
The purpose for collecting data, as and where appropriate, is for the delivery of Knowsley’s Integrated Sexual Health Service. Anyone accessing this service may be required to share information with the service provider for the purpose of delivering the service and supporting the individual accessing the service. Knowsley Council have commissioned Liverpool University Hospitals NHS Foundation Trust to deliver this service.
What allows you to use my information?
Knowsley Council commissions an Integrated Sexual Health Service in line with its statutory public health duties under the Health and Social Care Act 2012, which transferred responsibility for improving the health of the population to local authorities. Section 2B of the Act places a duty on each local authority to take steps to improve the health of its population, and the associated regulations (The Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch Representatives) Regulations 2013) identify the prevention, testing, treatment, and support relating to sexually transmitted infections, including HIV, as part of those functions.
Knowsley Council do not process any personal or special category data for this initiative. Liverpool University Hospitals NHS Foundation Trust are Independent controllers and have their own lawful basis for processing your information.
Liverpool University Hospitals NHS Foundation Trust will provide Aggregated and anonymous headline data to Knowsley Council that will identify trends and numbers, but will not identify individuals. This will be part of monitoring arrangements to support the delivery of the service.

What personal information will be processed?
Our commissioned service provider, Liverpool University Hospitals NHS Foundation Trust, will process the following personal information:
- Names (forename and surname)
- Addresses
- Telephone numbers (landline/mobile)
- Email addresses
- Health information
- Employment details and educational details
- Family details
- Visual images
- Behavioural information.
Who will my information be shared with?
Aggregated and anonymous data may be shared by the service with Knowsley Council as part of monitoring arrangements to support the delivery of the service. If required / appropriate, Knowsley Council may share the anonymous data with the commissioned service for HIV Advice and Support in Knowsley. The Council will not share the data with any other third party.
Do I have to provide this information and what will happen if I don’t?
No, individuals are not legally required to share information with the service. You can decide whether to share data or not with LUFT, and you can decide how much or how little you wish to share.
How long will you keep this data for and why?
Anonymous data will be held by Knowsley Council for a period of up to 5 years. Any personal data processed by Knowsley Council will be held in line with our Records Management Policy and Retention Schedule.
How will my information be stored?
Data held by Knowsley Council will be stored on a secure electronic system.

What rights do I have when it comes to my data?
Under the Data Protection Act (DPA) 2018/ UK GDPR, you have the following rights with regards to your personal data: -
- The right to subject access – you have the right to see a copy of the personal data that the council holds about you and find out what it is used for.
- The right to rectification – you have the right to ask the council to correct or remove any inaccurate data that we hold about you.
- The right to erasure (right to be forgotten) you have the right to ask the council to remove data that we hold about you.
- The right to restriction – you have the right to ask for your information to be restricted (locked down) on council systems.
- The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request.
- The right to object – you have the right to ask the council to stop using your personal data or to stop sending you marketing information, or complain about how your data is used.
- The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable).
