Who will own my data once I submit it?

Why do you need my information?

We will only have your information if you provide it – this will be to add your details to the list of people who are happy to be contacted to discuss their feedback further. If you do not wish to provide your information you can complete the survey anonymously.

What Personal Information do we collect?

In order for Knowsley Council Children’s Service to contact you to discuss the feedback that you have provided further we may collect all, or some, of the following information from you from this survey:

  • Name (e.g., surname and forename).
  • Contact details (e.g., mobile telephone number, landline contact telephone number).

We will only collect this information is you are happy to provide it. .

What allows you to use my information?

We are relying on your consent to provide your information, if you do not wish to do so, you do not have to.  Providing your name and contact details will act as an indication that you consent to the sharing of your personal data with us. You also have the right to withdraw your consent at any time, and we will delete your information. If you wish we will also delete your survey responses too.

Who will my information be shared with?

If you provide personal information in this survey, it will only be shared within the Children’s Transformation team within Knowsley Council.

Do I have to provide this information and what will happen if I don’t?

You do not have to provide any information you don’t want to. If you do provide your name and contact details and later want us to delete this information, you can email EarlyHelpDevelopmentTeam@knowsley.gov.uk and we will delete this from the survey responses.

How long will you keep this data for and why?

If you provide your information, we will keep this for six months – you may be contacted within this time to give further information about your responses to the survey. After six months, your information will be deleted.

How will my information be stored?

Your information will be stored in a secure SharePoint site – only those who need access to this information will have it.

Will this information be used to take automated decisions about me?

No, if you provide your information, it will only be used to contact you via phone to talk about your experiences accessing support through Knowsley Children’s Services.

Will my data be transferred abroad and why?

Your data will not be transferred abroad.

What rights do I have when it comes to my data?

Under the Data Protection Act (DPA) 2018/ UK General Data Protection Regulation (UK GDPR), you have the following rights with regards to your personal data: -

  • The right to subject access – you have the right to see a copy of the personal data that the council holds about you and find out what it is used for.
  • The right to rectification – you have the right to ask the council to correct or remove any inaccurate data that we hold about you.
  • The right to erasure (right to be forgotten) you have the right to ask the council to remove data that we hold about you.
  • The right to restriction – you have the right to ask for your information to be restricted (locked down) on council systems.
  • The right to data portability – you have the right to ask for your data to be transferred back to you or to a new provider at your request.
  • The right to object – you have the right to ask the council to stop using your personal data or to stop sending you marketing information, or complain about how your data is used.
  • The right to prevent automated decision making – you have the right to ask the council to stop using your data to make automated decisions about you or to stop profiling your behaviour (where applicable).
