Education Health Care Plan

An Education Health Care Plan is known as an EHCP. This is a legal document that describes a child or young person’s educational, health and social care needs, the support they need and the outcomes they wish to achieve.

The EHCP is reviewed every year and can stay in place if required until the young person turns 25. The local authority is responsible for updating the EHCP.

If support is still required once the young person turns 25, services will work together to ensure a smooth transition to adult life and make sure the young person receives the right support from adult services where needed.
