The Local Authority Common Application Form (CAF) is where you confirm your school preferences and provide application information about your child, but it may not request all the information that some schools need to apply the oversubscription criteria from their admission arrangements.
Therefore, some schools have a Supplementary Information Form (SIF) which collects extra information, specific to that school, that the school admission authority then use to apply their oversubscription criteria. A SIF must be completed in addition to the CAF, and a separate SIF must be completed for each school of preference that requests one.
It is the responsibility of the applicant to check whether they need to complete a SIF, or provide any other information directly to the school, and they can do this by checking the admission arrangements of the school/s.
All Knowsley Catholic and Church of England schools have a SIF that applicants should complete if they want their application to be considered under any faith category of the oversubscription criteria.
Applicants can obtain the SIF directly from the school/s listed below - for some Knowsley schools copies of the SIF are also available from the links to download and print.
A completed SIF must be returned directly to the individual school/s by the application closing date (31 October for secondary and 15 January for primary), or at the time of making an in-year application.
If a school is named as a preference on your application form, but a SIF is not completed, this could mean the application is only considered to meet a lower criterion of the school admission policy.
If a school SIF is completed, but the school has not been named as a preference on the application form, the school admission authority will not consider your child for a place. A completed SIF is NOT a valid application on its own.

If you are naming a school in another Local Authority as a preference on your application, check with the individual school if any SIF is required and complete and return it following their instructions.
