The common application form (CAF) you submit to the Local Authority to confirm your school preferences does not include all the information that some schools need to apply the oversubscription criteria from their admission policy.
Therefore, some schools have a Supplementary Information Form (SIF) which collects extra information, specific to that school, that the school then use to apply their oversubscription criteria. It is the responsibility of the applicant to find out whether they need to complete a SIF or provide any other information directly to the school by checking the admission policy of the school/s of preference.
Knowsley Catholic and Church of England schools have an SIF that applicants should complete if they want their application to be considered under any faith category of the schools' oversubscription criteria. SIFs are available from the individual schools or can be downloaded and printed from the links below.
A SIF must be completed in addition to the local authority application form, and a separate SIF must be completed for each school of preference that requests one.

Once you have completed your form
Completed supplementary forms must be returned directly to the individual school/s by the application closing date (31 October for secondary and 15 January for primary), or at the time of making an in-year application.
If a school is named as a preference on the Local Authority CAF but a SIF is not completed, the school preference will still be considered but this could mean the application is only considered to meet a lower criterion of the school admission policy.
If a school SIF is completed but the school has not been named as a preference on the Local Authority CAF, the school admission authority will not consider your child for a place.
If you are naming a school in another Local Authority as a preference on your application, you must check with the individual school if any SIF is required and complete and return it following their instructions.
