If you are a private hire driver and need to notify us of a change of:
- Name
- Address
- Telephone number
- Email address
- Operator
When completing this form to change your name or address, you must:
- Upload the front of your DVLA licence showing your new name or new address
- Upload your vehicle insurance certificate showing your new name or new address
- Pay the required fee
Receiving your new licence and badges
All licence documents will be issued as a PDF via email.
Your declared private hire operator, if they accept via email, will also receive a copy of the licence.
The email will contain instructions for collecting your badges.
