The Leaving Care Act 2000 outlines the
responsibilities and duties that Local
Authorities need to provide for care
leavers.
Every young person will be provided with a personal advisor who will work closely with the young person, their carer and their social worker. Personal advisors will:

- Prepare the young person for leaving care
- Provide advice and personal care
- Ensure each young person has an individual pathway plan including appropriate financial, accommodation, employment, educational and health related matters
- Review the plan on a regular basis
- Ensure that the young person has access to the Connexions service
- Remain in contact with the young person until the age of 21 or 24 (if, for example the young person is in full time education)
