Leaving care for young people

The Leaving Care Act 2000 outlines the responsibilities and duties that Local Authorities need to provide for care leavers.

Every young person will be provided with a personal advisor who will work closely with the young person, their carer and their social worker. Personal advisors will:

  • Prepare the young person for leaving care
  • Provide advice and personal care
  • Ensure each young person has an individual pathway plan including appropriate financial, accommodation, employment, educational and health related matters
  • Review the plan on a regular basis
  • Ensure that the young person has access to the Connexions service
  • Remain in contact with the young person until the age of 21 or 24 (if, for example the young person is in full time education)
