Hospital discharge
The Hospital Discharge Team offers a
responsive service to support people
who have recently been discharged from
hospital.
The team supports Knowsley residents to remain in their own home by identifying and providing the correct level of support. People can also access community resources to help them become more independent.
A care manager will assess patients in hospital and identify their particular needs. A referral will then be made to the Hospital Discharge Team and a care package requested. A start and end date of care package will be agreed, and a support worker will make an initial visit to your home to introduce the service.
Charges for the service will be arranged by social services and calculated on the basis of ability to pay. A member of the charging team will undertake a financial assessment in the service user’s home.
