If you need straightforward advice, information or support we will either provide or 'signpost' you to a suitable local organisation. This might be to help or support you to continue to live a healthy and fulfilling life. We will only explore your wider health and social care needs when necessary, and with your agreement.
Social Services, health and housing staff will always, where appropriate, work together to assess, plan, meet and review your care needs. The single assessment process should ensure that older people are not passed from one agency to another and can expect as much continuity as possible in the staff providing services.
Care services in Knowsley are becoming increasingly integrated. There is a community older persons team, which provides a single point of access to health and social services in the community for older people with complex needs and also provides access to therapy, pharmacy, home safety advice, health promotion and illness prevention advice.
