Health visitors
The Health Visitors service from the
Community Older Peoples Team provides
individual health assessments and care
packages to meet identified needs as
well as offering support to carers.
Health assessments can be offered to any person over 65 years of age registered with a Knowsley GP. Assessments can be completed at home and appropriate advice regarding health promotion and illness prevention are offered. A referral to other support agencies may then be made with the persons consent.
Referral to the Health Visitors service is via your GP or other health professional or locality well-being team.
